If your doctor has told you to watch your blood pressure and cholesterol -- join the club. But if you're trying to take better care of yourself, you'll find that writing down your blood pressure and exercise habits will help you spot trends and stay motivated to keep working toward your goals.

Print out a new page of this diary each week, and fill in rows of the table every day. The first time you fill one out, you should complete the top section as well so you have emergency contacts and information about your medications in one handy place.
